Control Change Cause Analysis (2002)

Brief description   
3CA is a method of root cause analysis, designed to assist the investigation of accidents of any type. It assists the investigator to identify significant  episodes within an accident and to structure inquiries into the reasons underlying them.

Development
3CA has its origins in a co-operative project run by Humber Chemical Focus and the UK Health & Safety Executive in 2000.  3CA was produced in response to a need for a root cause tool that  was quick to learn,  systematic, transparent and which produces results that are easy to communicate.

Download draft version of 2nd Edition (version 2.5, date 12 April 2008). In this new edition of the manual, the headings have been changed to simplify use and encourage the analyst to consider underlying causes.
