3CA: Control
Change Cause Analysis

3CA is a method of root cause analysis developed
with ease of training and universality of application in mind. Three forms of
the method exist, the manuals are maintained here: Form A
and Form B. From June, 2010, a graphical worksheet is
available; this can be used with the Form B manual, but a
dedicated manual (Form C)
is also available as a draft.

Observation (RSO) Studies
- Another document from the MORT canon; this 1976 monograph describes how
to systematically collect information from a members of the workforce about
their experiences. RSO is a variation on the Critical Incident Technique
methodology developed by Flanagan in the 1950s. The pdf
