3CA: Control Change Cause Analysis

3CA is a method of root cause analysis developed with ease of training and universality of application in mind. Three forms of the method exist, the manuals are maintained here: Form A and Form B. From June, 2010, a graphical worksheet is available; this can be used with the Form B manual, but a dedicated manual (Form C) is also available as a draft.

 
MORT: Management Oversight and Risk Tree

Using the original public domain sources, NRI has fully revised the MORT User's Manual and Chart and published a second edition of the materials.  These materials and the sources used (including the original research report by W.G. Johnson) are available here.

Risk Management Guide

This is one of the series of documents written as part of the MORT canon of literature. This 1982 edition is available for download as a scanned, text-searchable pdf file (14Mb). An extract containing the cover, forward and table of contents is also available as a pdf (445kb)

 
Reported Significant Observation (RSO) Studies
Another document from the MORT canon; this 1976 monograph describes how to systematically collect information from a members of the workforce about their experiences. RSO is a variation on the Critical Incident Technique methodology developed by Flanagan in the 1950s. The pdf file is text-seachable. (1.2Mb).
